滑車神經(英語:Trochlear nerve)是第4對腦神經,編號Ⅳ。[1]

滑車神經為運動性神經,起於中腦下丘平面對側滑車神經核,自中腦背側下丘方出;自腦發出後,繞過大腦腳外側前行,穿經海綿竇外側壁向前,經眶上裂,越過上直肌上瞼提肌向前內側行,進入並支配上斜肌

滑車神經是腦神經中最細的神經。

Axons from one of these subnuclei, the medial nucleus, decussate before exiting the nucleus itself.
