- title：優良或特色主題的名稱 （為討論頁使用）
- 例：喬治·巴頓，見：Wikipedia:優良主題/Example
- lead：即主條目的名稱，必須加入適當的格式和鏈結。避免加入任何粗體文字，且需與主題名稱相同。
- 例：{{classicon|GA}} [[喬治·巴頓]]
- count：此主題條目的數量。
- 例：3
- image（自選）：此主題選用的圖片。
- 例：GeorgeSPatton.jpg（見 Image:GeorgeSPatton.jpg ）
- imagesize（自選）本主題圖片的大小（以像素計），若此格填空，會自動將像素設定為60。
- 例：80
- column1, column2, column3：用這三格將主題裏的條目用項目符號一一列出，並適當地加入格式，鍊結與優特條目的標誌。
