上季说到：该从哪儿开始？Elena选择了Stefan而不是Damon，但观众发现Elena其实与Damon相遇在先。Rebekah将Matt和Elena的车逼到河中，Elena一名呜呼。她被Meredith使用吸血鬼的血“救活”。结果？她现在要么选择死亡要么转化成吸血鬼。与此同时，Bonnie帮助Klaus占据Tyler的身体，以保护他免遭“彻底死亡”的厄运。Alaric又死了。

官方剧情描述：事故发生后的第二天清晨，Elena从昏迷中醒来。她最害怕的事情已成现实——她死了，体内还残留著吸血鬼的血液。她现在要么选择死亡，要么经历可怕的转变过程，将自己变成一个吸血鬼。Stefan向Elena承诺，他和Bonnie会尽一切可能帮助她脱离现在的情形。Damon对Stefan不先救Elena而导致其死亡的做法十分恼怒，但Stefan坚称这是Elena的心愿。为了挽救Elena的命运，Bonnie决定付出高昂的代价。Damon将怒火发泄在侥幸逃生的Matt身上。看到Stefan和Elena深情款款、至死不渝的样子，铁石心肠的Rebekah竟然被感动了。Young牧师和镇理事会成员开始围捕吸血鬼及其追随者，导致一场出人意料的悲剧的发生。

● 本季的主题是：Elena如何去适应一个吸血鬼的生活，她是否能抵抗鲜血的诱惑，她如何得到“不违背良心”、“不杀生”的血源以及“Elena是继承Stefan的衣钵还是继承Damon的衣钵”。制片人Julie Plec暗示Matt可能是Elena的血源之一。

● Elena会记起Damon强迫她忘记的那两个重要时刻，Elena会与Damon谈论这件事，但“后果”不会立即显现，也不会改变Elena对Damon的看法。Elena和Damon的关系会有所“发展”，但那是一个长期而缓慢的过程。

● Damon需要一个新朋友，但那肯定不是Tyler。

● Meredith在第四季中有重要戏份。她不会成为Damon的爱情对象，但两人会保持稳定的“buddy”关系。
