义大利和美国有著热情友好的关系。自1840年以来，美国在义大利及其前身义大利王国设有外交代表处。然而，在 1891 年，意大利政府断绝了外交关系，并短暂考虑对美国开战，作为对未决案件的回应11 名意大利人在路易斯安那州新奥尔良被私刑处决，1941 年至 1943 年关系破裂，当时义大利和美国处于战争状态。

第二次世界大战后，意大利成为一个强大而积极的跨大西洋合作伙伴，它与美国一起寻求在冲突和国内冲突地区促进民主理想和国际合作。为此，意大利政府与美国合作制定国防、安全和维和政策。根据北约成员国的长期双边协议，在维琴察和里窝那（军队）有重要的美国军队驻扎；阿维亚诺（空军）；和Sigonella、Gaeta和那不勒斯——美国海军第六舰队母港。美国约有11,500名军事人员驻扎在义大利。意大利在罗马举办了北约国防学院。
